COVID-19 Vaccination Record
We understand that misplacing your COVID-19 Vaccination Card or needing documentation regarding your COVID-19 vaccines can be stressful. However, we're here to assist you with the necessary steps to obtain a replacement or access your COVID-19 vaccination history.
To achieve this, there two options based on your vaccination provider. If you received the COVID-19 vaccines through the Bridgeport Health Department, the process for requesting a replacement COVID-19 vaccination card or accessing your vaccination history may differ from those who received the vaccines elsewhere. Rest assured, we're here to help make the process as seamless as possible.
If the Bridgeport Health Department administered your COVID-19 Vaccine:
We can issue you a new vaccination card if you ever misplace or damage the original one. To obtain a copy, you will need to fill out our request form which can be found below. Once you've printed and filled out the form, simply bring it to our friendly staff at the Family Health and Wellness Clinic, along with a valid form of identification.
If a different provider, NOT the Bridgeport Health Department, administered your COVID-19 Vaccine:
You can get your immunization record through the CT WiZ Public Portal. CT WiZ is a free service provided by the Connecticut Department of Public Health Immunization Program
- Vaccine recipients and legal guardians can access official immunization records for themselves and their children, if they are under the age of 18, using the CT WiZ Public Portal with ‘two-factor authentication’ to verify identity.
- Note: Recipients won’t find the record if the contact information does not match what the provider reported to CT WiZ. If they are unable to locate their record in the portal, they can contact the Department Public Health Immunization Program by submitting a ticket through the help desk or calling 860-509-7929. Business hours are Mon-Fri 8:30 a.m. - 4:30 p.m.
