As a member of the Health Improvement Alliance (HIA), the Bridgeport Department of Health & Social Services is working with partners to distribute the DataHaven Community-Based Assets and Needs Survey (CBANS) to local residents. HIA is a coalition that formed in 2003 to address the health needs of Greater Bridgeport, which includes Bridgeport, Easton, Fairfield, Milford, Monroe, Stratford, and Trumbull.

The goal of this brief, voluntary survey is to better understand the quality of life and important issues facing people who live in our region. Your valuable input will help guide improvements to programs and services in our area, while helping organizations to understand where to focus their attention and resources. Completing this survey should take you about 5 minutes. All responses are completely anonymous and confidential. If you have any questions, you can email info@ctdatahaven.org.
